COMMUNITY ROOM USE AGREEMENT
With submittal of this application, the Applicant agrees to indemnify and hold harmless the City of Fort Morgan, its officers, agents, servants, and employees, from and against any and all claims or suits for bodily injury, illness, death, personal injury, or property damage (including, without limitation, reasonable fees and expenses of attorney's, expert witnesses and other consultants) arising out of any negligent act or omission committed by the Applicant, its officers, employees, and agents, in connection with the activities conducted under this agreement. This indemnification shall specifically include any claims arising from the negligence of the City, its agents, officials and employees. I have been given a copy of this receipt and a copy of all applicable rules governing this reservation and I understand these rules are herby incorporated by reference. I also understand nothing in this agreement is intended to waive governmental immunity.

COMMUNITY ROOM USE GUIDELINES
General information and rules to be followed when using the Community Room include:
1. Community Room in Library/Museum Complex is offered to the general public for meetings. It is not, however, for private social events, i.e., wedding receptions, graduations, birthdays, celebrations, etc.
2. Reservation requests are handled on a first come , first serve basis. Set-up and clean-up must take place during the time user has reserved the room. Meeting rooms may be scheduled up to 6 months prior to the reservation date. Verbal Reservations are strictly prohibited. A reservation request form MUST be filled out either online or in person at the Library/Museum Help Desk.
3. No running or unruly behavior allowed in the Library or Museum.
4. Food and drink are allowed in the Community Room. Catering is allowed but is the responsibility of the reserving party.
5. No orange or red drinks are allowed in the Community Room.
6. The kitchenette is not available for usage, with the exception of the sink.
7. No alcoholic beverages are allowed in/on City property.
8. Parents or guardians are responsible for their children.
9. Please do not misuse City items such as tables or chairs.
10. Users are responsible for cleaning up after their event. This includes vacuuming, and taking out all trash.
11. Library/Museum staff are required to be on-site during all times of use.
12. City staff has authority to discontinue user's access to the facility if any activity conducted during said use is illegal or deemed inappropriate. Library/Museum staff will advise the applicant to correct the problem. Failure to comply with this request could result in the City of Fort Morgan Police Department intervention.
13. All advertising, except that incidental to programs, and all sale of merchandise or other materials is forbidden on the premises. Organizations cannot charge a fee for any meeting held in the Community Room.
14. We reserve the right to cancel any reservation at any time by notifying the reservation holder. Any application, including those previously granted permission, may be rejected or suspended for the violation of the rules of conduct at the absolute discretion of the Director of Library & Museum Services or his representatives. Completing this reservation request form is verification that you understand and will adhere to the meeting room guidelines and rules.
15. Failure to appear within thirty minutes of your reservation will result in the cancellation of your reservation.
